Common Fixes
- Ensure graphics card is properly plugged in
- Ensure fans have no debris
- update drivers via the respective application
- test another PCI input on PSU
Advanced
- Clean drivers via respective application
- Reinstall the card
- Check for any signs of damage or overheating
- Ensure GPU dosent "sag" (is it properly latched ?)
- Ensure the PSU (power supply) provides enough wattage for the card
